Prequalification Scheme Application Form for Contractors

Please use the Department of Treasury and Finance Prequalification Scheme Guidelines for Contractors document to help you complete this form.

This form must be signed and submitted by a company director or an authorised representative.

Important: If you need to save a draft and return to the form later, or if you plan to complete this application collaboratively, do not log in. Instead, follow these steps:

Contact Details

The contact information you provide will be used for communication relating to the Prequalification Scheme, such as reminders of impending registration expiry and requests to provide an updated quality assurance accreditation certificate (where applicable).

These contact details will also serve as the nominated contact for Government Agencies.

We recommend that the email address supplied match your eTenders business user account registration, and that a shared mailbox be used to ensure important communication is not missed when personnel are unavailable.

Nominate the Prequalification Scheme category(ies) you are applying for. You must select either “Yes” or “No” for each category. Where you have selected “Yes” you must request a financial limit for that category.

Please note, contractors approved to the maximum financial limit of a category are permitted to undertake work exceeding that limit, except for those approved in the Building Works – Institutional category. For non-residential government building projects valued at $50 million or more, where prequalification is required, contractors must be prequalified under the National Prequalification Scheme (NPS). Further details are available on the Australasian Procurement and Construction Council (APCC) website.

 

Categories

Categories below reflect the categories selected on the Category nominations page.

Provide details of two completed projects for each category.

You can include additional projects in the Other Project Experience section at the bottom of this page.

All must have been completed within the past five years and should match the financial level you are applying for (either individually or combined).

We may contact you for more information.

Provide evidence of licences and Quality Assurance certification. These requirements are based on the Prequalification Scheme category(ies) and financial limits selected on the Category Nominations tab.

You must attach signed financial statements for the two most recent financial years, which have been prepared by an independent accountant or auditor.

It is important to ensure that financial statements are:

• independently prepared by an external accountant or auditor;
• final (draft statements or management accounts will not be accepted);
• full (i.e. include a Director’s Declaration, profit and loss statement, balance sheet and cash flow statement);
• signed by both the Director(s) and the accountant or auditor; and
• specific to the entity/ABN applying for registration.

If you are a subsidiary entity that does not prepare financial statements, please refer to the Financial Capacity Assessment section of the Guidelines for Contractors for further information.

 I / We hereby:

1. Declare that all information provided in this application is, to the best of my knowledge, true and correct and meets the requirements and conditions set out in the Guidelines for Contractors.
   
   
2. Acknowledge that the Treasury will seek information from third parties, including undertaking credit checks, as part of the assessment process.
   
   
3. Confirm that none of the proprietors, directors, trustees, managers or their spouses is bankrupt, or a director, manager or secretary of a company that is being wound up (whether voluntarily or otherwise), and that the business is not trading under:
   - an arrangement and/or restructuring; or
   - receiver and management; or
   - official management; or
   - an arrangement with creditors.
   
   
4. Agree to comply with and ensure that all employees, sub-contractors and agents comply with the requirements of the Work Health and Safety Act 2022, and the Work Health and Safety Regulations 2022, and all other Acts, regulations, local laws and bylaws, Codes of Practice and any other instructions made under any Act or subordinate legislation, whether State or Federal which are in any way applicable to the performance of the contractor’s services.
   
   
5. Undertake to promptly advise the Treasury of any relevant changes to the applicant’s circumstances or the information contained in this application and/or provided by the company accountant, including adverse changes to the financial position of the business, for the duration of the prequalification period should prequalification be approved.
   
   
6. Consent to the publication of select registration information in the Building and Construction Prequalification Register on the Winning Government Business section of the Tasmanian Government Purchasing website.
   
   
7. Acknowledge that prequalification does not guarantee that a contractor will be invited to undertake work for the Tasmanian Government.
   
   
8. Acknowledge that Treasury reserves the right to revoke, downgrade or suspend a contractor’s prequalification status based on new information obtained during the registration period.
   
   
9. Confirm that I / We are duly authorised by the applying entity to sign this application form.
